Service Options Using Resources in a Community Environment (SOURCE)

The SOURCE (Service Options Using Resources in a Community Environment) program is designed to assist individuals with chronic health conditions who have limited financial means and are unable to afford traditional nursing home care. The program’s goal is to enhance quality of life by providing essential nursing services in the comfort of the individual’s home—avoiding the need for placement in a hospital or long-term care facility.

Eligible participants in the SOURCE program receive care from skilled nurses either in their homes or within their communities. This approach not only promotes recovery but also allows individuals to remain close to their families and maintain a sense of normalcy in familiar surroundings.

Eligibility Requirements for the SOURCE Program

To qualify for the SOURCE waiver program, applicants must meet specific criteria established by the state.

Generally, individuals must be 65 years of age or older and have a chronic medical condition. However, individuals under 65 may also be eligible if they have a severe disability.
In addition, applicants must have full Medicaid coverage and meet all other program eligibility requirements.

Medical conditions commonly approved under the SOURCE program include:

• Respiratory and pulmonary disorders
• Diabetes
• Arthritis
• Heart disease
• Amputations
• Alzheimer’s disease
• Spinal cord injuries

Services Provided Under the SOURCE Program

Through the SOURCE program, our skilled nurses at URE Home Care deliver a range of services tailored to the individual needs and medical conditions of each patient. These services include:

• Light housekeeping
