Community Care Services Program (CCSP)

The Georgia Community Care Services Program (CCSP) is designed for seniors with limited financial resources who wish to remain in their homes rather than transition into a nursing facility. This program is a preferred option for many families, offering comfort, familiarity, and continuity of lifestyle.

URE Home Care is proud to be an approved provider under the Georgia CCSP Waiver, offering compassionate, in-home support to eligible individuals.

Who Qualifies for the CCSP Waiver in Georgia?

To be eligible for the CCSP, applicants must meet the following criteria:

• Be a resident of the State of Georgia
• Qualify for Georgia Medicaid
• Have a physical limitation that requires the level of care typically provided in a nursing home. Qualifying conditions may include arthritis, multiple sclerosis, visual impairment, amputations, Alzheimer’s disease, dementia, and related cognitive or physical impairments
• Choose to receive care at home rather than in a nursing facility
• Participate in only one waiver program at a time

What Services Are Provided Through the CCSP?

The CCSP helps seniors who are medically eligible for nursing home placement to remain in their homes, maintaining independence and a closer connection to their families. URE Home Care offers a comprehensive range of services under this program, including:

• Skilled nursing care, including wound care, physical, speech, and occupational therapy
• Personal care assistance with bathing, dressing, grooming, and eating
• Help with errands such as grocery shopping, meal delivery, bill payments, and more
• Light housekeeping services
• 24/7 emergency response support
